OverviewLooking for an Assessment Specialist for the Comprehensive Care Management Program to provide Case management, vocational rehabilitation and job preparation services for adults on public assistance with substance abuse issues.Responsibilities Responsible for assessing individuals referred to VNSNYs Comprehensive Care Management Program by NYCs HRA to determine work readiness and eligibility through a comprehensive substance abuse assessment and evaluation Determines exemption status, appropriate level of care and need for treatment, and refers client to treatment programs, also provides referrals to immediate services as needed Conducts comprehensive individualized substance abuse assessments on all individuals referred from New York City’s HRA and determines level of care based on clinical data obtained by reviewing clinical progress reports submitted to HRA by treatment programs and by utilizing HRA guidelines Evaluates and makes recommendations on treatment program transfer requests Utilizes NYCWAY/STARS systems to enter assessment outcome data, assign clients to work activities and/or treatment Arranges work activity and WeCARE appointments based on client need, assessment outcome and readiness for work as demonstrated in substance abuse assessment outcome Conducts comprehensive assessment of life and domain issues, strengths and barriers and incorporates into clients individualized assessment planQualifications Bachelors Degree from an accredited college or university in a health or human services related field such as rehabilitation counseling, social work, psychology or counseling, or equivalent work experience required Valid New York State Credentialed Alcoholism & Substance Abuse Counselor (CASAC) credential required Minimum of three years experience in a human/social services required Excellent oral, written and interpersonal communication skills required Proficiency with personal computers, including Microsoft Windows, Word required Bilingual skills (English and Spanish) required Valid New York State drivers license may be required as determined by operational/regional needs Mental Health assessment skills may be required, as determined by operational needs.

Knowledge of Human Resources Administration (HRA) Systems preferred Vocational Rehabilitation experience is preferred
