Well established Internal Medicine group practice is looking for Certified Nurse Practitioner on a full time/part time basis.
Practice has a great reputation serving the community for over 50 years.
It has an outpatient location in Poughkeepsie, NY and affiliation with Nursing Homes in the area and provides home visits on a selective basis.
Hours of operation are Monday to Friday from 8.00 AM to 5.00 PM, with 1 hour lunch break from noon to 13.00 PM and Saturdays from 8.00 AM to noon.
Candidate would be asked to work every third Saturday but the practice is flexible for the right candidate.
The NP will work in collaboration with the attending physician and will provide comprehensive health assessments and develop a plan of care for the patient population.
The NP will also document in the electronic medical record by recording subjective, objective data, assessment and plan records data in a consistent, legible, accurate, concise and timely manner.
He/she will provide patient/family education and utilize prescriptive authority working in collaboration with the attending physician.
The practice provides a great degree of autonomy with excellent physician back up.
The practice participates in many health care quality initiatives such as PCMH (Level 3), CPC and DSRIP.
REQUIREMENTS : Board Certified Nurse Practitioner Licensed to practice in NY Experience preferred but not mandatory.
The practice has worked with and has successfully oriented new graduates.
